The health declaration form must be filled out in the studio prior to the tattoo process.
According to regulation on hygiene requirements for tattooing businesses §7, we are obliged to provide you information about risks associated with getting a tattoo. To ensure that you understand what the service entails, you must read and fill out a health declaration in the studio before we can proceed with the tattooing.
The equipment used during the tattooing session is either disposable or sterilized. All work surfaces, tattoo machines, power supplies, wires, and grip bottles used during the tattooing session are covered with appropriate disposable packaging. The tattooing session is carried out according to the studio’s procedures.
The ink used during the tattooing session may contain nickel or chromium (VI) and may cause an allergic reaction. The tattoo artist only uses tattoo ink that complies with current laws and regulations governing tattoo ink.
I understand that tattooing involves piercing the skin and that infections are always a risk when getting a tattoo, and I agree to accept the risk that infections are possible.
I acknowledge that it is not possible for the studio or artist to determine whether I may have an allergic reaction to the pigments or procedures used on my tattoo, and I agree to accept the risk that such a reaction is possible.
I understand that a tattoo is a permanent change to my appearance, and that no promises have been made to later modify or remove my tattoo. To the best of my knowledge, I do not have any physical, mental, or medical impairment that may affect my well-being as a direct or indirect result of my decision to get a tattoo.
I understand that if I undergo any forms of skin treatments, laser hair removal, plastic surgery, or other skin-changing procedures, it may result in unwanted changes to my tattoo.
I understand that aftercare instructions will be given to me verbally and in writing via email or text message, and that I can also find aftercare instructions at www.inspirittattoo.no. I agree to follow the artist’s aftercare instructions while my tattoo is healing. I acknowledge that if any touch-ups are necessary due to my own negligence, I will be responsible for covering the cost.
I consent to the tattoo studio and/or tattoo artist taking pictures or filming the tattoo during the session and of the final result. The tattoo studio and/or artist have the right to use these images or films for marketing purposes. If I am not comfortable with or do not want my images or films used in this way, I must inform the tattoo studio and/or artist.
I understand that the artist may refuse to perform the tattoo session on me if I am under the influence of alcohol or drugs, if I do not provide the requested health information in this declaration, or if the tattoo could pose a risk to my own or others’ health due to my condition.
I consent to the studio and artist being released from any liability for any injury caused by me withholding health information requested by the studio or artist in this declaration regarding the execution of the tattoo or risks associated with getting a tattoo.
I understand that once the tattoo stencil is applied to my body, I am responsible for checking the stencil for any spelling errors and ensuring that it is in accordance with my wishes before the tattoo session begins. I am responsible for requesting changes to the design or placement of the stencil before the tattoo session begins, and I understand that such changes may affect the total price of the tattoo.
